Volunteer Application Information & Guidelines
A quick reference for the documents you'll need and how to prepare them before the mission deadline.

Required Documents by Volunteer Type
Look up your volunteer role below to see exactly what you need to submit:
Medical Professionals
Professionals who are either unable to or choose not to practice their specific profession are welcome to apply under a non-medical category.
- Doctor of Medicine (MD): Passport Info, Professional License, Specialty License (required for Hospital/OR, ICU, Radiology, and Minor Surgery; not required for general OPD), and PRC STP Form.
- Osteopathic Medicine (DO): Passport Info and Professional License. (No Specialty License or PRC STP Form required—please contact PMSNC to discuss your application processing).
- Nurses (All roles - OPD, Hospital, OR, Pre/Post Op, Med-Surg): Passport Info, Professional License, and PRC STP Form.
- Dental Surgery, Optometry, Pharmacy, Physical Therapist: Passport Info, Professional License, and PRC STP Form.
Podiatry & Chiropractic
- Podiatric Medicine / Chiropractic Medicine: Passport Info only. (No Professional License, Specialty License, or PRC STP Form required, as these are not separately regulated by the PRC in the Philippines).
Paramedical / Allied Health Services
(Includes: Dental/Optometry/Pharmacy/Triage Assistants, Education, OR/Bio-Med/SPD Technicians)
- Required: Passport Info only.
Non-Medical Volunteers
(Includes: Data Entry, Security, Photographer/Videographer, Students, Liaisons)
- Required: Passport Info only.
PRC Special Temporary Permit (STP) Instructions
If your volunteer position requires a Special Temporary Permit (STP), the online application system will automatically generate it for you. Once generated, download the form and follow these steps:
- Attach Photos: Affix to the application two (2) passport-sized photos with a complete name tag with white background positioned at the bottom of the photo.
- Gather Credentials: Prepare clean copies of your professional license and board certification.
- Prepare the Cover Letter/Affidavit: Download the official template available in PDF and DOC formats. Fill it out and print it. This single affidavit acts as an umbrella to certify all your attached credentials, saving you from having to notarize each one individually.
- Get Notarized: Have only the Cover Letter/Affidavit notarized. No other documents need notarization.
- Obtain an Apostille:
- California Residents: You can either submit your package to the PMSNC Medical Mission Committee or bring your documents to an in-person Volunteer Orientation, and PMSNC will assist with the California apostille.
- Out-of-State Residents: You must submit your complete document bundle to your respective Secretary of State to get an Apostille stamp.

Frequently-Asked Questions
- What should I submit to prove my professional credentials?
- Submit your medical board certificate. If you have a residency or specialized training certificate, include those as well.
- Do I need to write a formal letter to the Professional Regulatory Board (PRB) Chairperson?
- No. PMSNC handles all formal letters to the PRB Chairperson on behalf of all volunteers.
- What exactly needs Notarization vs. an Apostille?
- Notarization: Only the Cover Letter/Affidavit template.
- No Notarization: Do not notarize certificate copies or the main PRC form.
- Apostille: All documents in your packet (the notarized cover letter, professional license copy, board certification, completed PRC form, and the 2 passport photos) must be processed together for an Apostille stamp by your state's Secretary of State.
Passport Submission Guidelines
Who needs this? All non-Filipino citizens must submit a clear copy of their passport information page for clearance by the Philippine Consulate and local governments.
How to Take an Acceptable Passport Photo/Scan:
- Lighting: Use a well-lit area (natural daylight is best) to avoid shadows.
- Flat Surface: Lay your passport completely flat. If you use your fingers to hold the booklet open, make sure they do not cover any text or details.
- Angle: Hold your camera directly above and parallel to the page to avoid distortion.
- Clarity: Make sure the text is completely sharp. Do not use digital zoom (move your phone closer instead) and check for glare on the glossy laminate.
- Framing: Capture the entire information page without cropping any edges.

Professional & PRC Licenses
- No Valid License? If your professional license will not be valid at the time of the mission, you cannot apply for a licensed medical role. Please choose a non-medical or paramedical volunteer role that only requires a passport.
- Already have a Philippine PRC License? If you hold a valid PRC license, you will be prompted to upload a copy. Volunteers with a PRC license pay a reduced mission fee because they do not require expensive administrative processing, notary, or apostille services. Ensure your upload is clear, unblurred, and that no details are blocked out.
